Patient Policies
Non-Discrimination:
Seton Center’s nondiscrimination policy and procedures will meet specific requirements under the Uniform Grant Guidance, state and local laws, and best practices in grant management. This policy and procedure will also work in conjunction with Seton Center’s equal employment opportunity statement and policy. Seton Center has adopted requirements for participation in federally assisted programs and projects and shall implement such requirements regardless of the source of funds.
Patient Rights/Patients have the right to:
1. Respect and Dignity: Receive care that is considerate, respectful, and free from discrimination, coercion, abuse, or neglect and to be treated with dignity regardless of age, race, religion, sex, gender identity, sexual orientation, disability, or economic status.
2. Privacy and Confidentiality: Have personal health information handled confidentially in accordance with HIPAA law(s); Request restrictions on the use and disclosure of their medical information; Allowed to gain access, request amendments to, and get disclosure information about their health.
3. Information and Communication: Be informed in a clear, understandable manner about their diagnosis, treatment options or reasonable alternatives, prognosis, and cost of care;. Have access to interpreters or translation services if needed; Have the right to know the name of anyone providing care.
4. Participation in Care: Be involved in planning and making decisions about their health care, including refusing treatment (within legal limits); Have their surrogate decision-maker and/or legal guardian be involved in decision making; Receive a second opinion or request a change in Dental and/or Behavior Health provider; Allow a family member or other individual to be present and involved in decisions about care, as agreed to by the patient.
5. Consent: Receive complete information necessary to give informed consent, including risks, unanticipated outcomes, purpose, and duration before any procedure or treatment; Revoke consent at any time, where allowed by law.
6. Access to Care: Receive timely and appropriate medical care regardless of their ability to pay or source of payment.
7. Pain Management: Have their pain appropriately assessed and managed.
8. Advance Directives: Formulate advance directives and have those directives honored within the limits of the law and facility policy.
9. Complaints and Grievances: Voice concerns, complaints, or grievances about care or services without fear of retaliation; Receive a response to those complaints.

Patients are expected to:
1. Provide Accurate Information
2. Follow Treatment Plans developed by their Dental/Behavioral Health
providers, or accept the consequences of refusing care.
3. Respect Staff and Property
4. Financial Responsibility: provide necessary insurance information, fulfill unment financial obligations related to care and notify Seton of any changes.
5. Communicate: when they do not understand information or instructions and Inform Dental/Behavioral Health providers if their condition changes or worsens.

Seton Center will not allow any form of retaliation against team members, neighbors or patients who raise issues of discrimination. Any team team members, neighbors or patients that feel they have been subjected to any such discrimination or retaliation should contact the CEO and/or the Manager. To ensure the workplace is free of artificial barriers, violation of this policy including any improper retaliatory conduct will lead to discipline, up to and including discharge.
Our conduct council will review behaviors of neighbors, patients or other visitors to or facility that are not compliant with our Code of Conduct or Non-Discrimination Policy.
Seton Center is committed to complying with applicable federal, state, and local laws governing reasonable accommodations of individuals, including, but not limited to, the Americans with Disabilities Act (ADA) and the Pregnant Workers Fairness Act (PWFA). To that end, Seton Center will endeavor to make a reasonable accommodation to applicants and team members who have requested an accommodation or for whom Seton Center has noticed may require such an accommodation, related to an individual’s disability, religious beliefs and practices, needs as a victim of domestic violence, sex offenses, or stalking, needs related to pregnancy, childbirth, or related medical conditions; and/or any other reason required by applicable law, unless the accommodation would impose an undue hardship on the operation of our business.
Seton Center will not allow any form of retaliation against team members, neighbors or patients who have requested an accommodation, for whom Seton Center has notice may require such an accommodation, or who otherwise engage in the interactive dialogue process.
Reporting Procedures
If team members, patients or visitors have been subjected to or witnessed conduct which violates our policies, these individuals should immediately report the matter to Seton Center’s Conduct Council. Every report of perceived harassment will be fully investigated, and corrective action will be taken where appropriate. All complaints will be kept confidential to the extent possible. Retaliation is Prohibited. Violation of this policy including any improper retaliatory conduct will result in disciplinary action, up to and including termination.
