Seton Center & AccessKC
Seton Center can accept AccessKC and ThriveKC vouchers in our dental clinic to help lower or eliminate the cost of your care. We can also help you login to help you determine your eligibility for vouchers for ather services including vision exams, prescription glasses, medication, hearing aids, and durable medical equipment. See eligibility requirements below.
- Picture Identification for all household members 18+ Acceptable forms of Identification: Birth Certificate, Consulate Identification Foreign Driver’s license, Foreign Passport Certificate of Homeless I-94 card, Local Public School Enrollment card, Voter/National Identification card, Tax Documents
- Gross household income at or below 200% of current year’s Federal Poverty Level (FPL) guidelines—include all members of the household receiving income in the home
- Proof of residency in the greater KC area for the past 30 days
- Prescription, Medical Order or Letter of Medical Necessity Form from a physician’s office, clinic or other designated health professional validating a medical need. Durable Medical Equipment must have a statement from a medical provider within 90 days validating the medical need.
