Seton Center & AccessKC
Seton Center, in partnership with AccessKC, issues vouchers for emergency dental care, vision exams, prescription glasses, medication, diabetic supplies, hearing aids, and durable medical equipment. See eligibility requirements below.
- Picture Identification for all household members 18+ Acceptable forms of Identification: Birth Certificate, Consulate Identification Foreign Driver’s license, Foreign Passport Certificate of Homeless I-94 card, Local Public School Enrollment card, Voter/National Identification card, Tax Documents
- Gross household income at or below 200% of current year’s Federal Poverty Level (FPL) guidelines—include all members of the household receiving income in the home
- Proof of residency in the greater KC area for the past 30 days
- Prescription, Medical Order or Letter of Medical Necessity Form from a physician’s office, clinic or other designated health professional validating a medical need. Durable Medical Equipment must have a statement from a medical provider within 90 days validating the medical need.
