GENERAL JOB DESCRIPTION
Seton Center is a Kansas City-based human services nonprofit affiliated with Ascension Health, offering dental, social, and educational programs to underserved individuals and families. Seton Center is seeking a full-time Community Health Worker to serve as a liaison between clients and community resources, addressing gaps in healthcare access and social determinants of health. The goal is to improve wellness and self-sufficiency for all individuals served through Seton Center’s programs and the broader Kansas City community.
DUTIES & RESPONSIBILITIES
• May be assigned to work with a specific population served by Seton Center programs
• Educate community members and their families about the services and programs available at Seton Center
• Create opportunities for and facilitate group community health education sessions
• Support growth in program participation; work toward goals set by the Programs Director
• Attend community meetings, events, and activities on behalf of Seton Center
• Coordinate and participate in all Seton Center community events and outreach activities
• Build and maintain trusting, positive relationships with clients, internal teams, and social service agencies
• Maintain detailed, up-to-date records of all client interactions and activities affecting program productivity
• Conduct intake interviews to identify social service needs and connect clients to appropriate resources
• Coordinate and provide resources to address social needs in a manner that is safe, timely, effective, equitable, and client-centered
• Consult and collaborate with community systems to facilitate referrals and linkages to services
• Manage assigned caseloads; develop individualized resource plans, monitor case progress, and determine case closure
• Meet or exceed monthly case closure quotas as defined by program funders and the Programs Director
• Follow up with clients through phone calls, in-community visits, and home visits to track progress
• Record case information accurately; complete all required forms and produce statistical reports
• Submit weekly reports to the Programs Director by designated due dates
• Maintain events, training, and meeting details in the shared team calendar
• Develop working relationships with medical, social service, governmental, and community organizations
• Promote cost-effective and quality-driven interventions and outcomes
• Assist clients in scheduling clinic appointments and navigating health coverage programs as needed
• Attend staff meetings, trainings, and other meetings as requested
• Develop Memoranda of Understanding with partner community organizations as needed
• Maintain a high level of ethical conduct regarding confidentiality, dual relationships, and professional conduct
• Adhere to Seton Center’s professional standards, protocols, and mission
• Must be able to work some evenings and weekends
• May require certifications and further training to assist community members in accessing health programs or coverage (Seton Center willing to train)
• Other duties as assigned
PHYSICAL DEMANDS
This position requires physical activity including lifting, standing, bending, and twisting. The employee must be willing and able to travel locally for community outreach and client visits, and to work varying shifts including evenings and weekends to accommodate community and program needs.
WORK ENVIRONMENT
Work is performed within Seton Center’s facilities and out in the community through indoor and outdoor events, meetings, and client home and community visits.
MINIMUM QUALIFICATIONS
• High School diploma or equivalent required; Bachelor’s Degree preferred
• Minimum 1 year of experience in community outreach or working in a multicultural setting
• Bilingual candidates strongly encouraged to apply
• Medicaid Application Certification (Seton Center willing to train)
• Community Health Worker Certification (Seton Center willing to train)
• ACA Marketplace Certification (Seton Center willing to train)
• Must have reliable personal transportation, a valid driver’s license, and minimum current liability auto insurance
